INFORMATION ACCORDING TO THE LAW NO. 9887 DATED 10.03.2008 “ON THE PROTECTION OF PERSONAL DATA”, AS AMENDED
We hereby inform you that the processing of your data as part of the service you requested includes both personal data pursuant to Article 3 point 1, and even sensitive data falling within the special categories of data referred to in Article 7 of the Law no. 9887 dated 10.03.2008 “On the protection of personal data”, amended (hereinafter referred to as Law no. 9887). Your data will be processed in compliance with the principles of lawfulness, correctness and transparency, as well as for specific, explicit and legitimate purposes and, in any case, in a manner compatible with the same and that your personal data will be processed in such a way that they are “adequate”, “pertinent” and “limited” to what is necessary to achieve the provision of the requested service and “stored” in a form that allows the identification of the subject to which they refer only for the period of time which is necessary to achieve the purpose of treatment, while guaranteeing its integrity and confidentiality. The personal data you provide will be used solely for the purpose of fulfilling the service requested at our Clinic and in fulfillment of the obligations arising from the Law no. 9887, by regulations and legislation.
The processing of the data in question will be carried out both on paper and through computerized means, only with operations, as well as with such logic and forms of data organization, strictly necessary in relation to the obligations, tasks and to the stated purposes and, anyway, so as to guarantee its security and confidentiality. The data will be kept for a period not exceeding the period that is necessary to fulfill the obligations of the service contract and those arising from the law, regulations and legislation.
The data that will be required to be provided from you will be those strictly necessary for the fulfillment of the obligations deriving from the service requested at our Clinic or even just to be able to respond to your requests. The provision of such data is therefore indispensable and in the absence of the same it will not be possible for us to perform the requested service, where such data is indispensable.
It is your right at any time to obtain confirmation of the existence or otherwise of the processing of your data and to know their content and origin, verify their accuracy or request their integration, updating, rectification, cancellation or even the limitation in the case of processing in violation with the law, as well as transformation into anonymous form or to oppose the processing for legitimate reasons and, also, to revoke the consent at any time (including but not limited pursuant to Art. 12 – 17 of the Law No. 9887).
